We are enrolled with the following medical insurance providers: Medicare, Group Health PPO, United Healthcare, Cigna/Great West, Aetna, Humana, Premera, First Choice Health, Regence, Uniform Medical Plan, MultiPlan/PHCS, Railroad Medicare. As specific insurance policies may vary, please contact your insurance company or our office if you are uncertain if we accept your insurance.
We accept cash, check, VISA, and MasterCard. At the time of your visit, we require that you place a credit card on file with our office.
For patients without insurance and patients receiving cosmetic services, payment in full is due at the time of your visit.
For patients with insurance, your copayment is due at the time of service. We will bill your insurance directly for your visit. Once your insurance carrier processes your claim, your credit card on file will be charged up to $200 of the balance for that visit. We will then mail you a statement with any remaining balance due.
